Instructions for applying:- Complete the following on the Prerequisite Verification Form:
- Page 1: Complete sections 1-9 only; Skip to page 5
- Page 5: Answer question 41, sign and date the form
- Submit the completed Prerequisite Verification Form and a copy of your OSHA authorized trainer card to oshatraining@ucsd.edu .
IMPORTANT: Allow 10 business days for processing of submitted documentation. Prerequisite Verification Forms are being reviewed and processed based on the start date of the applicant’s selection.
